Filling out this form does not guarantee your use of the facility. Availability must be checked first, and an email invite will be sent once it is scheduled. Students requesting facility use, must have a staff member make a request and be present while using facilities.
All outside organizations:
Filling out this form does not guarantee your use of the facility. Availability must be checked first, and an email invite will be sent once it is scheduled with a copy of the completed form. All outside organizations must submit a certificate of insurance listing Bishop Unified School District as additional insured to use BUHS facilities. This will be required after request has been approved.
